Thank you for pursuing the highest quality practitioner for your dental and oral health needs. Your first visit will consist of a number of preventive services deemed usual by the American Dental Association. Insurance companies allow differing amounts for these services, and their coverage changes from year to year so it is impossible for us to keep up with both dental advances and insurance modifications. In order to maximize your benefit please contact your insurance company and ask them what your plan permits. The preventive or miscellaneous procedure codes you may receive are: D0150 - comprehensive oral evaluation, including oral / head and neck cancer screening. D1110 - adult prophylaxis (crown scaling and polishing) D0210 - complete peri-apical x-ray exam, (recommended every three to five years. A reduced fee is granted if acceptable films are provided at your appointment) D1310 - dietary counseling for prevention of decay D1330 - dental hygiene review, counseling or instruction D9310 - consultation to answer your questions If you have a health plan, you will leave the office with a completed claim form, in an addressed, stamped envelope. Payment is expected at the completion of each visit.
