New Patients:

Thank you for selecting this high quality practice for your dental and oral health needs.

There are two ways a new patient may be seen in this office. 

        Emergency treatment:  Patient is in pain or is visiting and has a sudden change in an oral condition.

        New Patient of record: Patient is ready to establish a doctor-patient relationship.

To establish the proper relationship. Your first visit will consist of a number of services deemed required by the American Dental Association, DC Law and accepted practice.   The preventive or miscellaneous procedure codes you may receive are:

D0150 - comprehensive oral evaluation, including oral / head and neck cancer screening. 

D1110 - adult prophylaxis (crown scaling and polishing)

D0210 - complete peri-apical x-ray survey, (recommended every three to five years. A reduced fee is granted if acceptable films are provided at your appointment)

D1310 - dietary counseling for prevention of decay

D1330 - dental hygiene review, counseling or instruction

D9310 - consultation to answer your questions

If you have a health benefit provider plan, you will leave the office with a completed claim form, in an addressed, stamped envelope.

Payment is expected at the completion of each visit.
